A primary issue to take into consideration for your breast enlargement surgery is the incision location for the breast implants. When you come to our San Jose area office for your consultation, Dr. Sutkin will consult with you on the choice of incision site, which will depend upon the particular breast surgery and your body type. There are primarily three types of incisions that are used with breast augmentation, breast lift, or any other surgery using breast implants: transaxillary (under arm), inframammary (in breast fold), or peri-areolar (around the nipple). Dr. Sutkin will help you determine which incision is best for you.
When discussing your breast enlargement surgery, another issue that should be addressed with your surgeon is the position of your breast implants. There are two methods of inserting breast implants: over muscle and under muscle. The advantages of placing the breast implants over the muscle are as follows: more cleavage, more natural movement, less pain (quicker recovery), droops more naturally with time, and no movement of implants with activity/exercise. The main disadvantages of inserting breast implants over the muscle are: the ability to feel the implant more (less tissue covering the implant), droops faster, greater risk of stretch marks, and more ripples if there is little soft tissue to cover the implant.
The other option for placement of breast implants is underneath the muscle. The advantages of this method are: less ripples, less movement of the implant, more visibility of breast tissue in mammograms, less droop with time, and less atrophy to the breast tissue over time. There are disadvantages to using under-muscle positioning for breast implants, which include less natural movement, a longer recovery time, and more discomfort following surgery. Another consideration with breast enlargement surgery is the shape of the breast implants. The two primary types of breast implants are round and anatomically shaped implants. Your surgeon can also help you determine which kind of texture the breast implants will have. Currently, most surgeons offer smooth or textured implants. With smooth implants, there is a more natural feel and less ridges and ripples. With textured implants, there is less movement of the implant and a lower risk of capsular contracture.
